THE GOAL of OUR NORTHGATE II PROGRAM is to REDUCE THE LIKELIHOOD of INAPPROPRIATE EMERGENCY ROOM UTILIZATION and LENGTHY HOSPITAL STAYS and ALLEVIATE or FORESTALL NURSING HOME PLACEMENT.
