ALL FACILITIES, SERVICES, PHYSICIANS and OTHER PROFESSIONAL STAFF of OSF HEALTHCARE SYSTEM SERVE ALL PATIENTS WITHOUT REGARD to RACE, RELIGION, AGE, SEX, NATIONAL ORIGIN, PAYER SOURCE or ABILITY to PAY. THE BOARD of DIRECTORS HAS ADOPTED FINANCIAL ASSISTANCE POLICIES and PROCEDURES WHICH APPLY for ALL FACILITIES and SERVICES of THE CORPORATION. THE AVAILABILITY of FINANCIAL ASSISTANCE is COMMUNICATED to PATIENTS IN NUMEROUS WAYS, INCLUDING USE of FINANCIAL COUNSELORS, PATIENT INFORMATION BROCHURES, and NOTICES ON PATIENT BILLINGS. FINANCIAL ASSISTANCE APPLICATIONS and INSTRUCTIONS ARE AVAILABLE ON WEBSITES MAINTAINED BY THE CORPORATION and UPON A REQUEST MADE to ANY of THE CORPORATIONS FACILITIES or OFFICES.
